DESCRIPTION
Secure Medical Data is comprised of highly trained and experienced professionals who have been in the medical transcription business for years. We pride ourselves in providing our clients with a quality service, guaranteed accuracy, on-time delivery, and highest level of confidentiality. Although we are a new provider, we have formed with some of the most experienced people in the industry and have built a service second to none.

SKILLS
Secure Medical Data specializes in medical back office management and electronic medical record (EMR) documentation and archiving. We are servicing physicians in all specialties, outpatient clinics, and primary to tertiary hospitals.

We will provide transcription services include but not limited to:
Web-based solution.
A 99% transcription quality.
Turn-around time within 4-24 hours or as requested by clients.
A 24/7 customer service.

EMR documentation and archiving is also available, together with medical billing and coding. Clients’ websites in three different packages are also part of the technologically advanced services that SMD is providing to healthcare practitioners and institutions.

TERMS
In setting up the client’s account, SMD needs the client’s email address. Rest assured that this will be kept confidential.

Clients will be billed every two weeks, Fridays, and will be faxed to the client’s office at or before 1500 EST. This invoice will cover Friday to Thursday of the same week. Clients need to settle all payments not later than ten (10) working days after the invoice has been issued.

Please note that clients will be given 24 hours, every day, after the final transcript has been submitted, to ask for corrections, revisions, or such concern, or the final transcript will be considered for billing otherwise.

As part of our customer service, we will give two (2) days’ trial of 30-minute dictation each day for our clients to have a feel of the workflow and to test the capacity of SMD transcription workforce in doing quality transcription.

Special or additional reports will be charged accordingly.

Minimum term of this contract:

There will be no minimum or maximum terms of contract as SMD assures clients satisfaction and convenience in their profession. Part of SMD goals is to give every reason to our clients that they will always sign with us for our service.

In the event of cancellation for any reason, please notify SMD office.

Late payments of fees will be assessed on any account past due by 15 days.

Final terms and conditions will be provided in the contract for services.
