TERMS
Terms and Fees: -

The initial term of the contract between our COMPANY and our CLIENT will be for 1 year after satisfaction of TWO months preliminary period in which we will charge you 5.0% after completion of preliminary offer we will charge 6.0% per month on net collection.

Net collections shall be defined as the total sum of all monies collected by our company for all clinical services rendered by CLIENT, less amount refunded or credited to patient or the third party payer as a result of overpayment, erroneous payment or bad checks. The Base Fee shall also be charged for collection agency recoveries.

In the event that CLIENT contrasts for the additional service of assistance with collection its OLD A/R, our company will provide a separate proposal and fee quotation that will apply to those services

We agree to make its internal practice, books and records relating to the use and disclosure of protected health information received from Health Care Provider, or created or received by our company on behalf of Health Care Provider, available to the Secretary of the United States Department of Health and Human Services for purposes of determining the Covered Entity's compliance with HIPPA (Health Insurance Portability and Accountability Act 1996).
